POSITION SUMMARY:
Community Habilitation Provider provides skill development training for a supported individual in and outside of individual’s home. Community Habilitation Provider must work effectively with the individual receiving services as well as family members and must provide a safe vehicle for transporting person receiving services during sessions.
*Up to $1000 in sign-on bonuses available- Earn $500 after 6 months of full-time employment and an additional $500 after 9 months of full-time employment.
POSITION QUALIFICATIONS:
Minimum Education: High School Diploma or equivalent required; college a plus.
Minimum Experience: Experience working with people with intellectual/developmental disabilities preferred.
Hourly Rate: $17.50
ESSENTIAL FUNCTIONS
- Train individual receiving services to develop/increase independence with verbal prompts, modeling and hand-over-hand assistance, as needed.
- Document training accurately and contemporaneously. Turn in billing and all documentation by Tuesday following each payday. Turn in Monthly Notes by the 15th of each month, for the previous month.
- Learn and utilize appropriate safeguards to ensure the supported individual’s safety and well-being.
- Communicate progress and concerns with family and supervisors regularly.
