POSITION SUMMARY:
Under the supervision of a Community Supervisor and/or a Lead Direct Support Professional, the Habilitation Counselor is responsible for providing habilitation supports in small group settings to people in varied environments, primarily the home and the community. This person is responsible for implementing programs as well as providing care to individuals. The Habilitation Counselor will provide a normalized, therapeutic, safe environment in the least restrictive atmosphere possible. The Habilitation Counselor promotes the positive self-esteem of individuals through consistent positive interactions, support, encouragement and respect of individual rights. The Habilitation Counselor provides instructional services to people receiving supports based on their interests and desires; and assists individuals in achieving individualized valued outcomes to increase skill development. The Habilitation Counselor documents the delivery of the habilitation activities on a daily basis, generates monthly summary notes and reviews required documentation to ensure compliance with OPWDD regulations and Agency policy. The Habilitation Counselor facilitates community inclusion activities on an individualized basis for each person; and is responsible for the safety of the individuals assigned to their care in all settings. Additionally, the Habilitation Counselor is responsible for assisting with the daily routine of the program, which may include: maintaining the physical environment, administering medications, meal preparation, providing supervision and hands-on assistance during meal times, lifting, transferring and assistance with activities of daily living (ADL’s), behavioral interventions, transporting individuals to medical appointments, as well as participating in recreation and/or social activities with individuals served. Assigned location may vary throughout the day, based on individuals’ needs. Depending upon specific assignment, this person may be required to work flexible hours, including nights and weekends.
The Habilitation Counselor demonstrates knowledge of The Arc Rockland’s mission, vision and values. In addition, this position actively participates in quality improvement projects (i.e. communicating issues, offering suggestions) to ensure on-going excellence of services, programs and staff to maximize the quality of life for individuals supported by The Arc Rockland.
POSITION QUALIFICATIONS:

Minimum Experience: A minimum of one year of experience working with individuals with developmental disabilities preferred.

- Must be 21 years of age or older
- Must have a valid and clean driver’s license for at least two years and successfully complete and maintain The Arc Rockland driver authorization

- Must successfully complete within six months of hire date and then maintain, certification for required training in Medication Administration (AMAP), Cardio Pulmonary Resuscitation (CPR), First Aid, and Strategies for Crisis Intervention and Prevention – Revised (SCIP-R), as required.
- Must successfully complete training on gastrointestinal nourishment assistance, ie. caring for G tubes, as applicable.
- Must successfully complete and maintain The Arc Rockland driver authorization.
- Must possess computer skills, ability to learn how to utilize the electronic health record and to complete virtual training requirements on electronic learning management system.
Physical Requirements: Occasional prolonged periods of walking or standing. Must have visual acuity adequate to read correspondence, computer screen, forms, etc.; manual dexterity; speaking and hearing ability sufficient to communicate by phone or in person at normal volumes. etc. Ability to physically assist and help lift individuals in and out of wheelchairs and vans. Ability to bend and stoop to assist individuals. Must be able to kneel and complete compressions to perform CPR if needed
Environmental Considerations: May be exposed to a few slip and trip hazards on the floor, odors, inclement weather, medical wastes, illness and unpredictable work conditions. May be required to work in areas within the agency, community and offsite locations that may or may not be heated or cooled.
Working Conditions: Work involves regular exposure with people receiving supports. Ability to physically assist and help lift individuals. Must drive agency vehicles, including vans, to transport individuals to appointments and community outings.
Corporate Compliance: Employees must comply with agency or governing body rules, regulations and policies and procedures. Any employee who witnesses or learns about a violation or possible violation of agency or governing body rules, regulations or policies and procedures is required to immediately report this to his/her supervisor or the agency’s corporate compliance officer.
