Hourly Rate: $24.00

POSITION SUMMARY: 

Under the direction of the Director or Nursing or Designee, the incumbent will provide direct nursing care by planning, organizing, documenting. and administering care provided to individuals who are supported by The Arc Rockland.  Care delivery will be done in compliance with rules and regulations of OPWDD and The Arc Rockland within the scope of the practice and assessed as such. The LPN will report to the Director of Nursing or Designee and may assist in all care provided to the individuals in Community Services programs.

POSITION QUALIFICATIONS:

Minimum Education: Graduation from an accredited practical nursing program.

Licensing/Certification: Active New York State LPN License. Current BLS certification. SCIP certification must be obtained and maintained.

Minimum Experience: One year experience working with developmentally disabled individuals as an LPN preferred.

ESSENTIAL FUNCTIONS:

  • Participate in Medication Management, including, but not limited to, administration of medication, maintenance of MAR’s, timely ordering of medications, the integrity and orderliness of the medication cart/room, and the appropriate disposition of outdated medications as per policy.
  • Maintain documentation as appropriate, including, but not limited to, accurate tickler files, medical records, appointment schedules and nurses’ notes.
  • Provide feedback and information and share concerns regarding supported individuals with the Director of Nursing or Designee.
  • Oversee and coordinate medical and dental appointments.
  • Participate in the formation of the Nursing Care Plan for each supported individual.
  • Participate in health training for supported individuals, including, but not limited to, recognizing signs and symptoms of medication side effects, disease process and management.
  • Monitor vital signs and response to medication and treatments.
  • Provide individual care as directed based on each individual’s Plan of Nursing Care.
  • Provide guidance, feedback and training to direct care staff based on needs of supported individuals.
  • Participate as directed in identification and prevention of abuse of supported individuals and identification of or provision of care in emergency situations.
  • Participate in health counseling based on the cognitive level of the individual – e.g., the importance of hand-washing in infection control.
  • Provide nursing treatments relative to restorative care and health maintenance, including, but not limited to: monitoring bowel and bladder training, tube feeding and wound/stoma care.
  • Monitor chronically ill individuals and report signs and symptoms of complications.
  • Participate in supportive care, including, but not limited to, chronic pain management, hospice, and palliative care.
  • Ensure that all individuals’ medical records are in compliance with all applicable state regulations and requirements of The Arc Rockland.
  • Support the interdisciplinary approach to individual care as well as team process and participate as directed by nursing supervisor.
