Salary: $60,000 – $65,000

POSITION SUMMARY:   Reporting to the Senior Director of Residential Services, is responsible for assisting with management supervisory oversight in multiple residences of the Agency and supervising Community Supervisors.  This includes but is not limited to operational, fiscal and disciplinary responsibilities such as assisting in hiring, training, evaluating, and terminating staff. Provides oversight to each individual’s programs. Ensures that supports are provided in a manner that is responsive to the interests and desires of each person. Works with families, advocates and other programs and providers of services to ensure appropriate service provision for individuals receiving supports. This position ensures the provision of appropriate individual care, protective oversight, that all safety and health requirements are met, and the general well-kept condition of the physical plant environment. Ensures compliance with all applicable regulations and Agency policies and assists with department development and expansion.


Minimum Education:  Bachelor’s Degree in a Human Services discipline or related field, Master’s Degree preferred; or five years of managerial experience in Human Service discipline preferred or equivalent experience with commitment to pursue degree

Minimum Experience:   Minimum of three years of pertinent experience required; or Master’s degree plus two years of related experience. Supervisory experience required.



  • Participates in all operational, managerial, regulatory and fiscal responsibilities in the running of the residences assigned under the direction of the Senior Director of Residential Services. Provides leadership and decision-making role for assigned programs. Acts as a role model for all subordinate staff.
  • Monitors the quality of all program activities, on and off-site, to ensure that person-centered services are delivered in response to the needs and interests of the individuals in the program. Directs the delivery of services in community-based settings and opportunities, based on capability and interest, to the greatest degree possible.
  • Directly supervises, mentors, develops and evaluates all assigned Community Supervisors to ensure they demonstrate competence in required job responsibilities.
  • Actively participates in the recruitment of, interviewing, hiring, scheduling, orienting, training, evaluating, mentoring, supervising, adjusting grievances or recommending same, disciplining and when necessary termination of subordinate staff. Ensures timely screening of all candidates for hire. Reviews and approves timecards for direct reports.
  • Provides oversight for all Habilitation planning, progress and evaluation in accordance with regulatory requirements. Reviews Staff Action Plans (SAP’s) to ensure most person-centered valued outcomes are being executed for each person supported. Ensures completion of SAP’s by deadline. Completes SAP’s as needed.
  • Prepares weekly billing reports, reviewing and ensuring that the daily attendance of individuals and documentation of service delivery is accurate. Assists in managing and ensuring accuracy of all data utilized for funding reports and timely submission of these reports.
  • Monitors the completion and quality of all monthly progress notes for assigned programs by deadline; ensures that all monthly progress notes are completed in accordance with agency policy and regulatory requirements; facilitates training as needed to improve the quality of the note content. Completes monthly progress notes as needed.
  • Participates in LifePlan meetings as needed. Works with Director of Documentation Mgmt. to ensure that there is a valid, current LifePlan for each individual, that accurately describes the habilitation services provided and includes person-centered supports; and to ensure any relevant changes to the SAP are accurately reflected.
  • Provides oversight to safeguard personal allowance funds for assigned individuals. Reviews financial records, ledgers, and receipts for assigned individuals/sites on a monthly basis. Ensures that Personal Allowance procedures are strictly adhered to in assigned programs. Reviews clothing allowance ledgers and receipts for assigned programs on a semi-annual basis.
  • Completes DDP-2 submission in CHOICES for all assigned individuals per regulatory guidelines (bi-annually). Obtains accurate information from Community Supervisor to do so.
  • Oversees the completion and submission of all incident reports in a timely manner as required by 624 regulations. Ensures all incident notifications are made within required time frames, including internal email notification, notification to advocates, etc., IRMA and OPWDD (as appropriate). Ensures all follow-up is completed within 30 days per agency standards and regulatory requirements. Acts as liaison for investigator.
  • Works with Agency QI and Compliance staff as well as OPWDD Department of Quality Improvement and/or other regulatory agencies to coordinate all regulatory reviews and to establish procedures to ensure compliance with recommendations and/or deficiencies. Assists Senior Director to complete all follow-up for recommendations and citations from all DQI reviews within 2 weeks. Assists Senior Director to complete all follow-up for recommendations from all internal quality and compliance reviews within 30 days.
  • Works with Senior Director to develop and implement combined Fire Evac / Site Pop

plan for each assigned home. Works with team to establish minimum staffing levels

for each home. Ensures all staff are trained on the plan timely. Conducts and/or

observes fire drills as assigned.

  • Ensures safe environment by performing regular physical plant walk-throughs, conducting facility safety checks, and completing work orders as needed. Reviews monthly physical plant, AED and water temperature checklists submitted by 15th of the month by Community Supervisors.
  • Reviews monthly vehicle mileage reports and ensures timely submission to transportation department. Follows up on related issues as needed. Ensures timely completion/submission of 5310 grant-related paperwork by community. Intervenes as needed with vehicle maintenance and repair issues.
  • Oversees implementation and delivery of program specific orientation for new hire, within accordance with agency policy. Assists Senior Director in ensuring that all program staff receive all required in-service and annual trainings in accordance with regulatory standards. Develops and implements in-service training related to needs of the people receiving supports.
  • Assists in formulating and recommending policies and procedures for department, long range plans for services and programs to be offered and implements as authorized.
  • Provides support to Community Supervisor in adhering to budgetary guidelines for assigned programs. Assists Snr Dir in administering overall budget. Provides support in management of over-time.
  • Reviews and approves Petty cash submissions for assigned programs. Ensures strict adherence to petty cash policies and procedures in assigned programs.
  • Acts as the designated community contact for HUD information as requested by Subsidized Occupancy Specialist.
  • Advocates on behalf of the individuals in relation to health care providers, local businesses, employers, transportation providers, social groups etc., when needed. Makes referrals and monitors all clinical services evidenced by current documentation in files and scanned into Medisked. Ensures individuals are living according to their rights and responsibilities and are free from abuse.
  • Visits each home regularly to become familiar with the needs of all individuals. Provides coverage and direct service provision as needed.
  • Assists Senior Director with recruitment, screening, and enrollment of new individuals for assigned residences.
  • On-call responsibility for assigned community on a 24-hour basis based on On-call structure.
  • Ensures that all internal, administrative, investigative reports, e.g. workers’ comp, drivers’ alert, accident reports, etc. are completed within specified timeframes.
  • Assumes all responsibilities of the Community Supervisor(s) in his/her absence. May be directly responsible to manage a home(s).
  • Temporarily assumes all responsibilities of the Senior Director in his/her absence.
