Position Summary
The Coordinator of Family Services / Social Worker performs work that ensures delivery of educational, clinical and social services to students and their families. This individual acts as a team member with staff, parents, and administration to facilitate the provision of a variety of social services to children and families. This individual handles outreach, intake, program/service referrals and the coordination of services for the Children’s Services division.

• Provides counseling as determined by the CPSE.
• Diligently and compassionately supports parents and families.
• Collaborates with PTFK staff, Department of Health, and School Districts regarding initial and supplemental evaluations.
• Serves as a liaison to foster consistent and coordinated communication between PTFK staff and families
• Oversees the student and staff assignments in CPSE Portal, works with school districts and RCDOH staff to ensure students are assigned correctly for monthly billing
• Consistently supports front office and staff with monthly attendance/billing
• Assists the office manager in the enrollment paperwork including emergency information and consent forms for all students
• Manages all aspects of the Prime-Time Lunch Program.
• Maintains daily data, communicates with CCSD, oversees auditing
• Assesses, refers, intakes families and individuals for services as needed through – CPSE.
• Provides tours to prospective students and their families
• Coordinates meetings with families and members of the treatment team for students attending Prime Time for Kids.
• A liaison between classroom staff and parents, providing support and insight
• Assists in the evaluation of the delivery of program services; provides methods/suggestions to improve program quality and assists in their implementation
• Analyzes current practices, brainstorms alternate suggestions for the program
• Assists director in searching for new services for students/individuals and maintains a directory of service list
• Serves as liaison with community-based agencies, including the DDSO and Rockland County Department of Health.
• Maintains up to date records and statistical information for Children’s Services as required by Federal, State, local authorities and our Agency.
• Assists in the development of and review of Children’s Services program plans.
• Provides training to staff as assigned by Director
• Performs related duties as requested
Requirements
Education & Credentials
• Master’s degree in Social Work or Psychology is required
• Certification or Certification Eligible as a New York State School Psychologist OR Licensed Clinical Social Worker
Experience
• One (1) or two (2) years of experience working with individuals with disabilities and their families.
Knowledge, Skills, & Abilities (KSAs)
• Fluency in English and Spanish, both spoken and written
• Strong written and verbal communication skills in both languages
• Ability to work effectively as part of a multi-disciplinary team
• Strong time management skills with the ability to handle multiple tasks efficiently
• Commitment to maintaining strict confidentiality
• Ability to meet paperwork and reporting deadlines accurately
• Physical ability and dexterity to work with small children, including stooping, kneeling, crouching, reaching, grasping, standing, lifting, and the ability to walk or run quickly
• Ability to occasionally lift children weighing up to 50 lbs
• Comfort working in situations that may involve stress
• Proficiency in operating a computer and using Microsoft Office Suite
